Can I order a test for someone else? (U.S Customers Only)

We recommend only purchasing the Heart Health test for yourself as you’ll be prompted to fill out a questionnaire (referred to as the intake form), before your order can be dispatched. This is to determine your eligibility to take the test. 

However if you need to order on behalf of someone else, please ensure that the person who is taking the test is the person who completes the questionnaire.
