I've been asked to fill out a questionnaire, why is this? (U.S Customers Only)


When placing an order for a blood test, you’ll be asked to fill out a short questionnaire before we can dispatch the testing kit. We use this to determine your eligibility to take the test. 

If we reject your order (and provide you with a refund), this would be based on the answers you’ve provided as this may indicate either:

  1. That you don’t meet the requirements to take this test, either due to age or biological sex.
  2. That you've indicated you’re experiencing symptoms that require immediate medical attention - we’d recommend visiting your local clinic, for the most appropriate care.
